29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Conveyor CX-3018A5 - An employee working on conveyor CX-3018A5 was exposed to caught-in hazards from not isolating all stored energy while clearing a tote jam, on or about December 12th, 2018. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
