29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On January 2, 2019, in the Pallet Assembly Building, two employees were exposed to a flying object hazard while using pneumatic nail guns to nail pallet wood pieces together in constructing a pallet without utilizing appropriate eye protection.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. The employer had not established and implemented an energy control program consisting of energy control procedures, employee training and periodic inspections.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes. a. In the Machine Building, the pulley to the shaker conveyor was unguarded and created an in-running nip point hazard to the operator and employee working in the area. b. In the Machine Building, the pulley to the Knotcher machine was unguarded and created an in-running nip point hazard to the operator. c. On the outside next to the Machine Building, saw dust blower area, the saw dust blower machine motor pulley was unguarded and created an in-running nip point hazard to the operator and the employee servicing the machine.
