29 CFR 1926.1402(b): The equipment was assembled or used without ensuring that ground conditions are firm, drained, and graded to a sufficient extent so that, in conjunction (if necessary) with the use of supporting materials, the equipment manufacturer's specifications for adequate support and degree of level of the equipment are met.  a.  On or about December 29, 2018, at the above addressed jobsite, an employee was operating a Lorain MCH-300 mobile hydraulic crane, and the crane tipped over from unstable ground conditions, thereby exposing the employee and employees from other trades to struck by hazards.

29 CFR 1926.1404(h)(2):  The size, amount, condition and method of stacking the blocking was not sufficient to sustain the loads and maintain stability.  a.  On or about December 29, 2018, at the above addressed jobsite, an employer was operating a Lorain MCH 300 mobile hydraulic crane, two outriggers were supported by inappropriate blocking materials (outrigger pads), such as but not limited to, scrap wood and single sheets of plywood, thereby exposing the employee and employees from other trades to struck by hazards.
