29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. FCA U.S. LLC located in Toledo, Ohio: On or about December 27, 2018, the employer did not ensure that the inpatient hospitalization of an employee who received a severe injury after being struck by a rotating shaft while operating a CNC lathe in the C Dock maintenance area was reported within 24 hours of the employer learning of the hospitalization.
