29 CFR 1910.1200(e)(1)(ii): Employer's written Hazard Communication Program did not address methods the employer would use to inform employees of the hazards of non-routine tasks: a) On or about January 3, 2019 the employer did not ensure that directions for conducting non-routine tasks during an employee's work schedule were included in the hazard communication program. Employees are tasked with running a barrel mixer to break down larger pieces of a product into a uniform size. The process is performed 2-3 times a year.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a.) On or about January 3, 2019, the employer did not train employees on the hazards of the chemicals they were working with. Employees working with the barrel mixer were exposed to hazardous materials including but not limited to; calcium silicon, which is combustible when in dust form.
