29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    On or about January 3, 2019 and times prior thereto, the employer had not developed or implemented a written respiratory protection program for employees required to wear full face respirators when painting and finishing wooden cabinets and other products. This condition exposed employees to inhalation hazards.    The program shall include the following elements:       (e) Medical Evaluation       (f) Fit testing       (g) Use of respirators       (h) Maintenance and Care of Respirators       (j) Identification of filters, cartridges, and canisters       (k) Training        (l) Program Evaluation

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:    On or about January 3, 2019 and times prior thereto, the employer had not gone over the information in Appendix D with employees who are allowed to voluntarily wear N-95 dust masks while cutting wood in the main shop area. This condition exposed employees to inhalation hazards.
