Employees used the walkway frequently to access the work area exposing employees to a fall hazard.

On or about and times prior to: 1/4/2019   Location: stairs to main walkway ramp to Old plant turn-head room  a) The employer used an steel L-shape angle bar with a approximate thread depth of 4" exposing employees to a trip hazard.

29 CFR 1910.28(b)(1)(i): 29 CFR 1910.28(b)(1)(I): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling:   On or about and times prior to: 1/4/2019    Location: Old plant turn-head room    a) The employer did not provide a fall protection system or adequate floor covering to prevent employees walking and working near an unprotected edge exposing employees to a fall hazard which resulted in an employee falling approximately 8 feet through a floor opening.

Employees were exposed to fall and engulfment hazards due to inward angled sides and variable depth dependent on aggregate demand.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about and times prior to: 1/4/2019  Location: Main walkway ramp to Old plant turn-head room  (a) The employer did not provide conveyor belt guards or roller guards underneath an operating conveyor belt. Employees are exposed to a rotating parts, caught-in and in-running nip-point hazards where a person passing underneath may contact moving parts, such as, return idlers and overhead conveyor belt less than 7 ft. clearance.
