OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards:    The employer failed to protect workers from struck by hazards associated with the set-up and operation of a Columbia Industries low profile landfill tipper (Serial # 073471).  This was most recently documented on or about January 4, 2019, at the worksite located at 16984 3rd Street in Lawrence, Kansas.  The employer did not ensure methods were in place to ensure pedestrian safety when tractor-trailers, without clear view to the rear, when driven in reverse.     Acceptable means of abatement include but are not limited to:    a) Ensure truck drivers and landfill tipper operators have an effective, agreed upon, communication method such as hand signals.     b) Ensure truck drivers maintain eye contact with the tipper operator while backing the truck/trailer.     c) Ensure truck drivers and other equipment operators do not use mobile communication equipment for purposes other than communicating work activities while on landfill property.    d) Ensure the back-up alarms are provided and working on truck/trailers backing onto the tipper.
