29 CFR 1910.22(a)(3): The employer did not ensure walking-working surfaces were maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. (a) Located in the above ground fuel tank area, metal electrical conduit crosses the walking-working surface, exposing employees to trip and fall hazards. IN THE ALTERNATIVE: 29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. (a) Located in the above ground fuel tank area, access to the working area was obstructed by metal electrical conduit, exposing employees to trip and fall hazards. NOTE: Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.304(b)(3)(i): The employer did not ensure that all 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms or on rooftops had ground-fault circuit-interrupter protection for personnel. (a) Located in the bathroom of Fuel Depot Building 2262, employees were exposed to hazards associated with electric shock and fire, in that the outlet below the A.O. Smith Water Heater did not have a ground-fault circuit-interrupter protection. NOTE: Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
