29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area: a) On or about January 2, 2019, employees were exposed to fire and/or explosion hazards where training did not include measures employees could use to protect themselves from the presence of hydrocarbons on well sites where vacuum truck water extraction operations were taking place . The employer did not ensure that employees were equipped with gas monitoring and detecting equipment while engaged in the process of transferring production water from the site tank to the vacuum truck.

29 CFR 1910.1200(g)(10): The employer did not ensure that, in all cases, the required information was provided for each hazardous chemical, and was readily accessible during each work shift to employees when they were in their work area(s): a) On or about January 2, 2019, the employer did not provide the required information to employees relating to the production water that was being transferred onsite. The Safety Data Sheet (SDS) provided by the employer was for water that was a by-product of crude oil production and not natural gas. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
