29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:  a)128 Main St., Westfield, MA: On or about December 17, 2018, employees were observed throwing debris (from roofing) off the roof of this building without any chute to the ground which was more than 25 feet below the roof eave.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a)128 Main St., Westfield, MA: On or about December 17, 2018, an employee was observed roofing this residence more than 25 feet above the ground below without any fall protection.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).   Steve Ribeiro DBA Structural Home Improvements was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 (b) 13), which was contained in OSHA inspection number 1146124, citation number 1, item number 1 and was affirmed as a final order on 7/1/2016 with respect to a workplace located at 80 Greenacre Ave., Longmeadow, MA.
