29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: a) Job site, 10955 Perry Highway, Wexford, PA: On or about December 31, 2018, during construction activities, the employer did not ensure than an employee working in an aerial lift used an appropriate fall protection system/device while operating an aerial lift in that an employee did not use any type of fall protection while in the basket working at a height of approximately 10 feet, allowing the employee to be ejected from the basket and hit the ground. b) Job site, 10955 Perry Highway, Wexford, PA: On or about January 3, 2019, during construction activities, the employer did not ensure than an employee working in an aerial lift used an appropriate fall protection system/device while operating an aerial lift in that an employee wore a harness and used a shock absorbing lanyard while working in the basket at a height of approximately 17 feet, which would have allowed the employee to hit the ground had he been ejected.
