29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about January 04, 2019, at the jobsite located Highway A & Joseph Street, across the street from Veterans of Foreign Wars, Hawk Point, Missouri, an employee for Arc Grading, LLC was observed working in an excavation that measured approximately 6 feet in depth, 2 feet in width, and 40 feet in length with only one ladder. The ladder was located such that it may have required more than 25 feet of lateral travel for a safe means of egress.
