29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:       (a)    A Team Roofing & Construction, Inc. at 420 Main St., Briggsdale, CO 80611:  On and before January 4, 2019, the employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during roofing operations. This condition exposed employees to the hazard of projectiles to the eyes.        (b)   A Team Roofing & Construction, Inc. at 415 Leslie St., Briggsdale, CO 80611:  On and before January 4, 2019, the employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during roofing operations. This condition exposed employees to the hazard of projectiles to the eyes.

(a)    A Team Roofing & Construction, Inc. at 415 Leslie St., Briggsdale, CO 80611:  On and before January 4, 2019, the employer did not ensure their employees were protected from injury, in that about five employees engaged in roofing activities on top of a commercial building were not utilizing a fall protection system. This condition exposed the employees to a fall hazard of approximately 15 feet.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      (a)    A Team Roofing & Construction, Inc. at 415 Leslie St., Briggsdale, CO 80611:  On and before January 4, 2019, the employer did not ensure that the ladder employees were using to access the upper landing surfaces extended 3 feet above the upper landing area. Five or more employees engaged in roofing activities on the roof of a commercial building did not have their extension ladder adjusted correctly so that the side rails extended at least 3 feet above the work surface. This condition exposed the employees to a fall hazard of approximately 12-13 feet.
