This condition exposed the employees to a fall hazard of approximately 15 feet.

Five or more employees engaged in roofing activities on the roof of a commercial building did not have their extension ladder adjusted correctly so that the side rails extended at least 3 feet above the work surface. This condition exposed the employees to a fall hazard of approximately 12-13 feet.
