29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warehouse: On January 7, 2019 and at times prior, employees were exposed to struck-by hazards while working in and around a Toyota Forklift Truck Serial No. 60615 with fork extensions while transporting and elevating goods and materials. b) Warehouse: On January 7, 2019 and at times prior, employees were exposed to struck-by hazards while utilizing an in-house fabricated man basket attached to the forks of a Toyota Forklift Truck Serial No. 60615 to lift personnel.
