29 CFR 1910.1096(d)(2): Appropriate personnel monitoring or equipment such as film badges, pocket chambers, pocket dosimeters of film rings were not supplied to or required to be used by employee(s) as described in 29 CFR 1910.1096(d)(2)(i),(d)(2)(ii) and (d)(2)(iii):    (a) Friends Animal Hospital, LLC - The employer did not provide film badges to the employee(s) taking x-rays of the animals, on or about January 7, 2019.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)  Friends Animal Hospital, LLC - The employer did not maintain a written hazard communication program for the employees using Chlorox Healthcare (hydrogen peroxide) and Parvosol II RTU (diethylene glycol monobutyl ether, tetra sodium ethylenediamine tetra acetic acid), on or about January 7, 2019.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
