29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On December 20, 2018 at approximately 4:30pm, an employee working with a backhoe received an amputation of the tip of his finger. The amputation was not reported within 24 hours to the local Area Offices of the Occupational Safety and Health Administration. The hospitalization was reported by phone on December 28, 2018 at approximately 4:30pm.
