29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  a.  On or about January 8, 2019, on the rooftop, employees operating Hitachi and Bostitch pneumatic nail guns to perform re-roofing work were not wearing eye protection being exposed to flying debris or nails.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a. On or about January 8, 2019, on the east side of the building, employee performing re-roofing work were exposed to fall hazard while ascending and descending from a ladder that was not extended three feet above upper landing level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about January 8, 2019, on the rooftop, employees performing re-roofing work from a 6:12 pitch roof were not being protected from a 9 feet and higher fall hazards by a fall protection system.   Southern Coast Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1229482, Repeat citation number 1, item number 1 and was affirmed as a final order on June 28, 2017, with respect to a workplace located at 5703 Fargo Drive North, Jacksonville, Florida.
