29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting: a) Facility: The employer did not provide training to employees expected to use portable fire extinguishers in the event of an incipient stage fire on the use of the extinguisher or the hazards associates with fighting an incipient stage fire, on or about 1/7/2019 BY THIS DATE THE EMPLOYER MUST EITHER CORRECT THE ALLEGED VIOLATION(S) OR IMPLEMENT A FIRE SAFETY POLICY, AS OUTLINED IN 29 CFR 1910.38 WHICH INCLUDES THE EVACUATION REQUIREMENTS OF 29 CFR 1910.157(b) NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) BP Print Group located at 315 Fourth Street in Lakewood, NJ: On or about 10/24/2018, an employee was cleaning the ink rollers on the AB Dick offset press when the tip of his finger got caught by the ingoing nip points of the ink rollers. The incident resulted in the amputation of the tip of the employee's pointer finger on the right hand. The employer did not report the amputation to OSHA.
