29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   a) BP Print Group located at 315 Fourth Street in Lakewood, NJ: On or about 10/24/2018, an employee was cleaning the ink rollers on the AB Dick offset press when the tip of his finger got caught by the ingoing nip points of the ink rollers. The incident resulted in the amputation of the tip of the employee's  pointer finger on the right hand. The employer did not report the amputation to OSHA.
