Location: 680 Bloomfield Ave., Montclair, NJ. a) An employee hoisting windows to the upper level used a Genie aerial lift that was equipped with a makeshift wood fork. The makeshift fork and/or modification was not certified by the manufacturer; on or about 1/7/19.
