29 CFR 1926.453(a)(1): Aerial lift was not designed or constructed in accordance with the applicable requirements of the American National Standard for "Vehicle Mounted Elevating and Rotating Work Platforms," A92.2-1969.  Location: 680 Bloomfield Ave., Montclair, NJ.    a) An employee hoisting windows to the upper level used a Genie aerial lift that was equipped with a makeshift wood fork. The makeshift fork and/or modification was not certified by the manufacturer; on or about 1/7/19.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were sitting or climbing on the edge of the basket.  Location: 680 Bloomfield Ave., Montclair, NJ.    a) An employee hoisting windows climbed the basket of a Genie aerial lift at the upper level.  The employee was exposed to a fall hazard of up to approximately 15 ft. to the lower level; on or about; on or about 1/7/19.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    Location: 680 Bloomfield Ave., Montclair, NJ.    a) An employee engaged in window delivery activities used a Genie aerial lift without the use of a personal fall arrest system; on or about 1/7/19.
