29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: At Aisles "M" and "N": On or about 01/08/2019, and at times prior, the employer exposed employees to falls of up to fourteen feet-three inches, in that personal fall arrest equipment was not provided to employees while dismantling storage racks.

29 CFR 1910.30(a)(3): In that employees had not been trained in the nature and recognition of fall hazards in the work area; the procedures to minimize those fall hazards; the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; the correct use of personal fall protection systems and equipment.
