29 CFR 1910.333(a)(1): Live parts to which an employee was exposed were not deenergized before the employee worked on or near them: ESTABLISHMENT: The BB1 machine was not deenergized and lockout while an employee was performing assembly of a prototype machine to install a power distribution box into the machine. On or about 12/14/2018, an employee inserting a power distribution box unevenly came in contact with the backwall resulting in a short circuit and an employee sustaining an electrical shock.
