29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. Location: Kitting Area While setting up the foam Cutting Machine, employees were exposed to amputation hazards by reaching past the guard to unjam the material and pull it into position to start producing lengths of material without unplugging the cord and de-energizing the machine.
