29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about January 7, 2019, on a 6:12 pitch roof, an employee performing re-roofing operations was not protected from a 40-foot fall hazard by use of a personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about January 9, 2019, employees were using a 24-foot Werner extension ladder to access a 2nd story landing improperly, as the ladder did not extend 3 feet above the platform and had a missing rung, exposing employees to a fall hazard.    b. On or about January 9, 2019, employees were using a multi-purpose (Little Giant) set up on the 2nd floor balcony to access the 2nd story roof that was not tied-down, exposing employees to a fall hazard.   c. On or about January 9, 2019, employees were using a 12-foot section of an extension ladder to access the top of a roof improperly, as the ladder did not extend 3 feet above the eave, exposing employees to a fall hazard.    d. On or about January 9, 2019, employees were using an 8-foot a-frame ladder on the 3rd floor balcony to access the 3rd floor roof that did not extend 3 feet above the eave, exposing employees to a fall hazard.

On or about January 9, 2019, employees using extension ladders at the jobsite were not trained to recognize, control or minimize the hazards associated with ladder usage.
