29 CFR 1910.140(c)(10)(i): Personal fall protection snaphooks and carabiners must not be connected directly to webbing, rope, or wire rope; unless they are designed for such connections: On or about 1/3/2019, at Area 7 of a warehouse located in Miami, Florida, employees erecting a shelving rack system were wrapping the webbing of the shock-absorbing lanyard around a upright and then attaching the snaphook of the lanyard directly to the webbing itself. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent: On or about 1/3/2019, at Area 7 warehouse located in Miami, Florida, the employer did not ensure tags were used to inform all workers were aware the shelving racks installation was completed, which an employee fell 22.5ft to the ground level when he stepped on a metal decking that was not secured to the shelving unit. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
