29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:   a) Job site, 131 Locust Street, Emsworth, PA: On or about January 9, 2019, during roofing activities, the employer did not ensure that a safe means of accessing a ladder jack type scaffold was provided in that the pick was set up on the outside of the ladder supports, causing an employee to climb up/over the pick to get up/down, exposing him to a fall hazard of up to approximately 20 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    a) Job site, 131 Locust Street, Emsworth, PA: On or about January 9, 2019, during roofing activities, the employer did not ensure that an employee had scaffold training prior to working on a scaffold. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection and not providing a safe means of access/egress to the scaffold, both of which exposed the employee to fall hazards of up to approximately 20 feet.

Deficiencies noted during the inspection included, but were not limited to, bent/damaged rungs and labeling that was marred, missing or illegible.
