29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. Location: Jobsite On or about January 9, 2019 employees were exposed to fall hazards of up to 28 feet to the ground below by using a fall restraint system anchorage point that was attached to a roof truss that was not designed to except a 5000 pound lateral load.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: Location: Jobsite On about January 9, 2019 employees were exposed to fall hazards of up to 28 feet to the ground below, while working on a roof where the 3/8" wire rope being used as part of an anchorage point for a personnel fall arrest system, was not inspected prior to use and had wear on it consistent with rigging and lifting and was not rated for a 5,000 pound load.
