29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: Employees engaged in grain handling activities in the grain elevator at the entrance to the tunnel floor were exposed to 91 inches (7.58 feet) fall hazards in that the walking-working surface next to the stair leading to the boot pit had an opening of 39 inches. The employer did not ensure employees on the walking-working surface of the entrance to the tunnel floor were protected from falling by guardrail systems, safety net system, or personal fall arrest system. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees engaged in maintenance activities in the Ag shop were exposed to injury in the event of wheel failure in that the distance between the left side adjustable tongue guard and the abrasive wheel periphery measured 0.5 inch for the Professional #1766 - 6" Bench Grinder. 29 CFR 1903.19(c)(2): Abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: Employees working in the feed mill boiler room were exposed to electrical shock hazards. Strain relief was not provided, in that the electrical conduit connected to the water softener tank was detached at the end connection, thus exposing energized wires. 29 CFR 1903.19(c)(2): Abatement of the above violation was verified at time of inspection, no certification is required.
