The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) On or about 01/09/2019 - 2718 Wenonah-Oxmoor Rd (Lot #1), Birmingham, AL, an employee was exposed to fall hazards of approximately 10 feet above ground while installing shingles and using a positioning belt at the wearer's waist as fall protection system.
