This was most recently documented at 21825 I St., Elkhorn, Nebraska 68022 on or about January 9, 2019. The employer did not ensure that a supported scaffold platform that was being used with a powered industrial truck was secured to the forks. Abatement certification and abatement documentation is required for this violation. The abatement certification sheet is enclosed.

The employer is failing to ensure that employees were using a ladder for the purpose for which it was designed. An employee was using a portable A-frame ladder to access a mobile scaffold while in a closed position. Abatement certification is required for this violation.
