29 CFR 1926.1101(k)(9)(vi): Training for employees performing Class IV operations did not include available information concerning the locations of thermal system insulation and surfacing asbestos containing material and/or presumed asbestos containing material, asbestos-containing flooring materials, or flooring material where the absence of asbestos has not yet been certified; and instruction in recognition of damage, deterioration, and delamination of asbestos containing building materials; and did not take at least 2 hours:  Jobsite: On or about November 13, 2018, the employer did not adequately train employees performing Class IV operations on the locations of presumed asbestos containing material (flooring material) and instruction in the recognition of damage, deterioration, and delamination of asbestos containing building materials.

29 CFR 1926.1101(o)(4)(ii): For Class III and IV asbestos work, the competent person was not trained in aspects of asbestos handling appropriate for the nature of the work, to include procedures for setting up glove bags and mini-enclosures, practices for reducing asbestos exposures, use of wet methods, the contents of this standard, and the identification of asbestos:  Jobsite: On or about November 13, 2018, on a job site where Class IV asbestos work was performed the competent person was not trained in the aspects of asbestos handling for the nature of the work to include practices for reducing asbestos exposures, use of wet methods, the contents of this standard, and the identification of asbestos.
