OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a. On or about January 10, 2019, and at times thereto, the employer did not ensure that the bridge travel pendant controls were uniform among the four cranes in operation in the building, thereby exposing employees to a crushed-by hazard. Crane 4 was in operation with travel controls operating opposite the direction of the other cranes.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a. On or about January 14, 2019, and at times thereto, employees were using multiple spreader beams in the building to lift precast structures with the cranes. The employer did not ensure that the load ratings were marked on the beams, exposing employees to a crushed-by hazard while loading the beams.

29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service. a. On or about January 14, 2019, the employer did not ensure that all slings, chains, and lifting beams were being inspected on a daily basis prior to use.
