29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)   This violation occurred on or about January 10, 2019 at a job site located at 1804 Ramsgate Road, Forney, TX; where employees were working and walking around on the ground and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  (a) This violation occurred on or about January 10, 2019 at a job site located at 1804 Ramsgate Road, Forney, TX; where employees were using a 24-feet extension ladder that did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a) This violation occurred on or about January 10, 2019 at a job site located at 1804 Ramsgate Road, Forney, TX; where employees were placing roofing shingles on top of a two (2) stories single family home and were exposed to a fall hazard to the ground level.   ABEL FIGUEROA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUMPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(13) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1231523, CITATION NUMBER 1, ITEM NUMBER 1A THAT WAS ISSUED ON MAY 16, 2017 WITH RESPECT TO A WORKPLACE LOCATED AT 1115 BANTHAM WAY, FORNEY, TX 75082.  THE FINAL ORDER DATE WAS MAY 16, 2017.
