29 CFR 1910.269(c)(1)(i): A more extensive job briefing was not conducted where an employee could not be expected to recognize and avoid the hazards involved in the job: (a) Unit 10 Switchyard - An employee was not properly briefed to know his assignment was to tape up ground caps on the de-energized side of the transformer. Instead he accessed the energized side and was exposed to an arc flash; on or about 01/07/19. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $ 1,000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.269(d)(3)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: (a) Unit 10 switchyard - The low side of the transformer was energized. The doors to LB91 were not locked out or tagged to prevent access by an employee who was exposed to live parts; on or about 01/07/19. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $ 1,000.00 AS PER 29 CFR 1903.19.
