29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Employees are exposed to struck by hazards from the electric pallet jack. This was most recently documented on or about January 9th, 2019 when the employer did not provide the electric pallet jack operator with training consisting of formal instruction, and practical training and evaluation. At least one pallet jack operator had not had training including a written and practical evaluation of the pallet jack operator's operation of their equipment.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Employees were exposed to electrical hazards. The extension cord being used to plug in the electric pallet jack did not have adequate strain relief. The wiring had pulled loose where the electrical cord met the plug-in exposing interior wiring.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv): Employees were exposed to chemical hazards. Employees had not received training on hazard communication regarding the chemicals they work with and where the safety data sheets are located. Employees use chemicals such as; CBI Ultra Concentrated V.O.C. Free Glass Cleaner when can cause serious eye damage/eye irritation; Nyco Balance EC Neutral Floor Cleaner that can cause skin corrosion/irritation, and Chemstar Sterilox Fresh Produce Maxx that can cause moderate eye irritation and is corrosive to metals.
