Employees are exposed to struck by hazards from the electric pallet jack. This was most recently documented on or about January 9th, 2019 when the employer did not provide the electric pallet jack operator with training consisting of formal instruction, and practical training and evaluation. At least one pallet jack operator had not had training including a written and practical evaluation of the pallet jack operator's operation of their equipment.
