29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about January 10, 2019:  At the worksite located at 4409 Bellhurst Lane Lot #19 Cumming, Georgia 30040, employees installing roofing material were exposed to serious injuries when the employer failed to ensure that each employee exposed to fall hazards were properly attached to lanyards at the wearer's back.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufactures specifications.   On or about January 10, 2019:  At the jobsite located at 4409 Bellhurst Lane Lot #19 Cumming, Georgia; where the employer failed to ensure the material hoist ad the proper type rope installed where the prescribed wire type rope was replaced with a nylon "climbing" rope used to lift materials to the roof area exposing workers below to potential failure if the rope were to break or the knot loosen.
