On or about January 10, 2019:  At the jobsite located at 4409 Bellhurst Lane Lot #19 Cumming, Georgia; where the employer failed to ensure the material hoist ad the proper type rope installed where the prescribed wire type rope was replaced with a nylon "climbing" rope used to lift materials to the roof area exposing workers below to potential failure if the rope were to break or the knot loosen.
