29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 01/11/2019 employees at the 7-Eleven construction project were exposed to eye punctures.  Employees were operating power equipment without eye protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 01/11/2019 employees at the 7-Eleven construction site were exposed to approximately 15' falls. Employees were engaged in framing activities without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  Employees at the 7-Eleven construction project were exposed to approximately 15' falls.  Employees ladder did not extend at least three feet above the landing.
