Walnut St., Springfield, Missouri- Employees were observed removing materials off of a forklift and were observed standing on a pile of debris that was stacked as high as the window sill. The employees were observed standing on the sill and window opening to remove the materials and were not protected from falling by the use of any means of fall protection.  The DDG Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number  1326171, citation number 1, item number 1,  and was affirmed as a final order on August 8, 2018, with respect to a workplace located at 2313 S. 63rd St., Omaha, NE 68106.
