29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Worksite located at 430 E. Walnut St., Springfield, Missouri- Employees were observed nailing and stapling down Tyvek paper on the exterior of the building and were not protected from eye injuries by the use of safety glasses.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift:   Worksite located at 430 E. Walnut St., Springfield, Missouri- Employee was observed working in an aerial lift and had failed to attach a lanyard to the boom or basket of the lift to prevent an employee from being catapulted out of the lift.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  Worksite located at 430 E. Walnut St., Springfield, Missouri- Employees were observed removing materials off of a forklift and were observed standing on a pile of debris that was stacked as high as the window sill. The employees were observed standing on the sill and window opening to remove the materials and were not protected from falling by the use of any means of fall protection.  The DDG Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number  1326171, citation number 1, item number 1,  and was affirmed as a final order on August 8, 2018, with respect to a workplace located at 2313 S. 63rd St., Omaha, NE 68106.
