29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Building M-32: On or about 01/10/2019, the path to ground for an aluminum recessed light was not continuous and permanent in that the ground wire was not connected at the sliced joint. b) Building M-32: On or about 01/10/2019, the path to ground for the Metal clad cable used with temporary lights was not continuous and permanent in that the Metal Clad cable was not connected to an enclosure that was grounded. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(b)(1): Openings in boxes, cabinets, and fittings through which conductors entered were not effectively closed: a) Building M-32, 3rd floor, 120 volts breaker panel: On or about 01/10/2019, the bottom section of the enclosure had (4) four openings that were not provided with covers as a result exposed employees to a possible arc flash fire. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
