29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)      Professional Contractors, on or about January 10, 2019 at the address of 815 Marlon Circle Marion, OH: The employer did not ensure employees were protected from falls while performing framing activities for a residential type structure.  Employees were exposed to up to 14 falls to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)      Professional Contractors, on or about January 10, 2019 at the address of 815 Marlon Circle Marion, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while installing a new dormer on the front side of a residential structure. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 14ft.

Professional Contractors, on or about January 10, 2019 at the address of 815 Marlow Circle Marion, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:     a.) Professional Contractors, on or about January 10, 2019 at the address of 815 Marlon Circle Marion, OH: The employer did not assure that a personnel platform used to lift employees was secured to the lifting carriage of the rough terrain fork lift.  The employees were standing on the personnel platform while being raised up to the roof to construct a dormer on the front of the residential structure.
