29 CFR 1904.30(a): The employer did not maintain a separate OSHA 300 Log for each establishment that was expected to be in operation for one year or longer. a) Caffes-Steele: Inc.: On or about 01/10/2019, a separate OSHA 300 log was not maintained for the Charles City location. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) Norfolk Naval Shipyard Building M-32: On or about 01/10/2019, a 50-foot orange extension cord used to supply power to a circular saw was ran between a heavy wooden door and metal sill plate without protection from possible damage. No abatement certification or documentation required.
