29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Outrigger scaffold, 2nd floor rough window opening, favoring the west side at the rear of the home: Employees without any fall protection, fell approximately 24 feet to the ground, when one of the bearers for the scaffold failed, or about 01/09/2019. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.452(i)(1): The inboard end of outrigger beams, measured from the fulcrum point to the extreme point of anchorage, shall be not less than one and one-half times the outboard end in length: a) Eastern most beam for the outrigger scaffold, 112 E Texas Ave, 2nd floor, rear of home: The inboard end of the failed outrigger beam was anchored approximately 32 inches from the fulcrum point; outboard end length was approximately 42 inches, on or about 01/10/2019; b) Western most beam for the outrigger scaffold, 112 E Texas Ave, 2nd floor, rear of home: The inboard end of the failed outrigger beam was anchored approximately 32 inches from the fulcrum point; outboard end length was approximately 72 inches, on or about 01/10/2019. 29 CFR 1926.452(i)(3): The fulcrum point of outrigger beams shall rest on secure bearings at least 6 inches (15.2 cm) in each horizontal dimension: a) Eastern outrigger beam, 2nd floor, rear of home: The outrigger beam was attached by screws to the vertical, eastern 2 inch x 4 inch rough opening framework for the window, approximately 1.5 feet above the bottom, horizontal framework for the window, on or about 01/10/2019.
