29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59):  At the residential construction site located at 2004 NE 114th Street in Kansas City, Missouri: Employees engaged in framing activities were exposed to health hazards from dermal, ingestion, and inhalation exposure to hazardous chemicals.  The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work.  Hazardous chemicals used at the site included but were not limited to Kwik Kaulk.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the residential construction site located at 2004 NE 114th Street in Kansas City, Missouri: Employees were exposed to struck by hazards. Framer was observed installing wood materials using a nail gun without eye/face protection.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   At the residential construction site located at 2004 NE 114th Street in Kansas City, Missouri:  Employees engaged in construction activities were exposed serious fall injuries, in that a Skytrak R50 lift was being used to support an elevated makeshift work platform. The platform was a flat platform that was not specifically designed and approved by the manufacturer for this task.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:   At the residential construction site located at 2004 NE 114th Street in Kansas City, Missouri: Employees engaged in construction activities were exposed serious injuries while working from an elevated makeshift work platform over 10 feet above a lower level without the use of Fall Protection. Employees were working from sheets of plywood covering the forks of liftApproximately 13 feet above the ground with no form of protection.
