Employees are conducting roofing work on a residential construction site at heights in excess of 6 feet.     This was most recently observed at or about 3814 George B Lake in Omaha, NE new home construction site on January 11, 2019.  The employer is failing to ensure that all employees are protected from fall hazards when performing roofing operations.

Employer was failing to ensure that portable ladders being used to access upper landing surface extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Employee was exposed to the potential falling hazard due to ladder(s) not being extended the required 3 feet above the landing surface. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.    The employer is failing to protect employees from electrical hazards associated with flexible electrical cords with signs of strain relief in the insulation and had been pulled away from the plug.  The strain relief was pulled away from the plug coming off of the 3 way splitter electrical cord on site.  After discussion this was removed and stowed.    This was most recently observed at or about 3814 George B Lake Parkway Elkhorn, NE 68022 on January 11, 2019.
