29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: At a commercial jobsite, the employees working below other employees working from a tubular welded frame scaffold were not weraring hard hats and exposed to falling objects and head injuries.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access. At a commercial construction jobsite, employees performing masonry activities from a tubular welded frame scaffold were not provided with a ladder for safe access and/or egress and used the cross-braces and end frames to access different working levels on the scaffold, exposing employees to fall hazards of approximately 40 feet to the ground below.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. At a commercial construction jobsite, on the south side of Building 5, the tubular welded frame scaffold had inadequate guardrails on the scaffold; the ends were left open and were not guarded to protect employees performing masonry activities from a fall hazards of approximately 40 feet to the ground below.
