29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) On or about January 7, 2019 and times prior; on the Atom feeder roller: The employer failed to ensure the employee feeding the leading edge of a small paper roll through the feeder had the electric and pneumatic energy sources turned off and locked out to prevent the employee from getting caught in the roller nip point.
