29 CFR 1926.451(h)(2)(i): Where there was a danger of tools, materials, or equipment falling from a scaffold and striking employees below, the area below the scaffold was not barricaded to prevent employees from entering.  On 21 December 2018, a piece of cinder block fell off a scaffold platform from five levels high during dismantling, striking a worker below. The area around the scaffold was not barricaded to prevent access, exposing workers in the area to the hazard of being struck by falling materials, tools, and debris from a height of approximately 32 feet 10 inches.
