29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: The employer is failing to protect employees from slip and fall hazards in the workplace. Employees are having to walk across sidewalks outside of the facility that are not free of ice causing them to slip and fall. This was most recently documented at 1600 N. Harrison St. O'Neill, Nebraska on or about January 7, 2019 where an employee fell and fractured hip as the employee was walking on the icy sidewalk.

This was most recently documented at 1600 N. Harrison St. O'Neill, Nebraska on or about January 14, 2019. An employee working for the company slipped on ice on the sidewalk and was hospitalized on December 31, 2018.
