29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). Note: The service entry power line (220 volts AC) was less than 3 feet away from this scaffold.    a) On or about 01/14/19, at a roofing jobsite on a 2-story house at 2591 Genesee St., Buffalo, NY: A Ladder Jack Scaffold was erected on the east side of the house towards the front of the house. Two employees were standing on the aluminum pik of that scaffold while doing roofing work on the bottom of the front gable. An energized 220 volt-ac overhead electrical power line that connected to the service entry panel of the house actually contacted the front aluminum support ladder of the scaffold. At one point, one of the employees was standing on that aluminum support ladder.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 01/14/19, at a roofing jobsite on a 2-story house at 2591 Genesee St., Buffalo, NY: A Ladder Jack Scaffold was erected on the east side of the house towards the front of the house. Two employees were standing on the aluminum pik of that scaffold while doing roofing work on the bottom of the front gable. The employees were exposed to a fall of approx. 15 ft. because no fall protection system was being utilized.       b) On or about 01/14/19, at a roofing jobsite on a 2-story house at 2591 Genesee St., Buffalo, NY: A Ladder Jack Scaffold was erected on the east side of the house towards the back of the house. An employee was sitting on the aluminum pik of that scaffold while doing work on the roof eave. The employee was exposed to a fall of approx. 22 ft. because no fall protection system was being utilized.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about 01/14/19, at a roofing jobsite on a 2-story house at 2591 Genesee St., Buffalo, NY:  Three employees were working on the ground doing clean-up work on the east side of the house in the vicinity of employees working above on a Ladder Jack Scaffold. The employees were  exposed to the potential hazard of being struck by materials that could fall off the  scaffold. The employees were not wearing a hardhat.        NO ABATEMENT CERTIFICATION REQUIRED
