29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, was not worn and attached to the boom or basket when working from an aerial lift: a) Jobsite: On January 15, 2019 and at times prior thereto an employee was not wearing a personal fall restraint system while performing tree trimming activities from a Reach-All 5000 series aerial lift truck. The employee was exposed to an approximate 30 foot fall. Note) Certification of corrective action is required for this item.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) Jobsite: On January 15, 2019 and at times prior thereto employees performing tree trimming activities were not protected from head strikes and overhead hazards. An employee working out of the bucket on a Reach-All 5000 series aerial lift truck and employees working on the ground below, were not wearing protective head gear or hard hats. Note) Certification of corrective action IS required for this item.
